The prevalence of diabetes mellitus in Indonesia in 2013 was 2.1%. This figure is higher than in 2007 (1.1%) .The prevalence of DM in East Java experienced an increase from 2007 by 1.8% to 2.5% in 2013.Method: The design of this study used a qualitative method with a CSR approach (Case Study Research). Data validity test uses source triangulation with domain analysis in data analysis. Participants were taken as many as 3 participants, namely the patient's family, patients with diabetes mellitus and home care nurses. The research data was collected at the participant's house in the Lowokwaru village of Malang in July 2019.Results: The research data were collected using interviews and observation methods.
