  • Background: Fixed orthodontic appliances which commonly used to treat malocclusion can complicate the patients in maintaining oral hygiene and thus facilitate the attachment of dental plaque. Plaque is a biofilm mainly consists of bacteria which can cause caries and periodontal disease and must be prevented by doing plaque control. Banana peels have antibacterial properties such as flavonoids, tannins, saponins, and terpenoids. The aim of this study was to evaluate the effectiveness of banana peel extract as a mouthwash to prevent plaque accumulation. Method: This study type is quasi-experimental using pretest and posttest with a control group design involving 30 fixed orthodontic patients. Subjects were divided into two groups, experimental group was given banana peel extract mouthwash and control group was given aquadest. Subjects were educated to apply Charter toothbrushing method for seven days before they were intructed to use the mouthwash or aquadest according to the group twice a day for seven days. Attin plaque index was measured before and after seven days of mouthwash application. Result: Attin plaque index scores after mouthwash application showed that there was a significant differences between banana peel extract mouthwash group and aquadest group. Conclusion: Banana peel extract mouthwash is effective to reduce dental plaque index of fixed othodontic patients. Key Words: banana peel, dental plaque index, fixed orthodontic appliances
