  • Introduction: Prediabetes is a condition when blood glucose levels increases from the normal range and below the diagnostic threshold of diabetes. Prediabetes has a high potential for progresiveness into DM type 2. Early diagnosis of prediabetes and diabetes helps on selecting of the exact intervention, so that it can decrease the incidence of complication. This research was conducted to assess the accuracy of the TG / HDL ratio as a marker of insulin resistance that is more efficient, applicable and affordable. The purpose of this study is to assess the sensitivity, specificity, AUC, cut of point, PPV, NPV, LR +, LR - from TG, HDL and TG / HDL ratio. Method: This study uses data from the Integrated Haj Computerization System in Health (SISKOHATKES 2018). The number of participants are 5,564 people who met the criteria inclusion. The obtained data is analyzed by using STATA version 15 and MedCal. The result of diagnostic test is presented in the form of table and curve form of each marker, namely TG, HDL, TG / HDL ratio and it is compared to find marker that have high diagnostic test values. Results: Diagnostic values of each prediabetes marker is TG level with value cut of point > 80, sensitivity 84.3%, specificity 24.5%, AUC 0.55, LR+ 1.12, LR- 0.64 , PPV 84%, NPV 90%. HDL level cut of point value ≤57, sensitivity 69.5%, specificity 33.5%, AUC 0.50, LR+ 1.04, LR- 0.91, PPV 85% and NPV 86 %. TG / HDL ratio cut of point value> 1.46, sensitivity 80.8%, specificity 25.4%, AUC 0.54, LR+ 1.08, LR- 0.75, PPV 84% and NPV 87%. Conclusion: Based on research that has been conducted on Indonesian pilgrims, it can be concluded that the TG / HDL ratio can be as a marker as a prediabetes screening method because it has a high level of accuracy with value of cut of point > 1.46, sensitivity 80.8%, specificity 25, 4%, P = 0,000 (significant), AUC 0.54, LR+ 1.08, LR- 0.75, PPV 84% and NPV 87%.
