Gotu kola plants contain many chemical component that have nootropic and neuroprotective effect, such as asiatic acid, madecassic acid, asiaticoside, dan madecassoside. Gotu kola has the potential to improve memory, stimulate nerve growth, and inhibit the action of acetylcholineesterase enzymes in the hydrolysis of acetylcholine neurotransmitters. Gotu kola plant has the potential to be an alternative nerve supplements or smart drugs. This research aims was to determine the efficacy of gotu kola water extract compared to donepezil in increasing acetylcholine level in the brain of healthy Wistar rats.&#xD; Method: In vivo experimental research with pretest and posttest control group design has been conducted in animal house and biomolecular laboratory of Medical Faculty Sriwijaya University on August-December 2017. Gotu kola is extracted by decoctation method. Gotu kola water extract was administered orally 2 mL/rat with feeding tube for 21 days with a dose ratio of 200 mg/kgBW, 400 mg/kgBW, and 800 mg/kgBW. Donepezil 5 mg/kgBW is used as positive control and water as a negative control. Performed measurements of acetylcholine of the brain by ELISA. Phytochemical tests were performed to determine the active chemical compounds in the extract. Data analysis was done by using SPSS version 24.&#xD; Result: Gotu kola water extract with dose of 200 mg/kgBW can significantly increase cerebral acetylcholine level compared with comparison groups. Only in the 200 mg/kgBW dose group there was a significant difference in increasing acetylcholine level compared with donepezil.
