The type of this study was cross sectional, with total sampling technique (n = 45), the sample is all PIS-PK cadres in Puskesmas Gandus. The result of univariate study showed that most of the cadres who actively participated in the program (PIS-PK) (48.9%), were highly educated (55.6%), old age (51.1%), low knowledge (75.6%), low motivation (51, 1%), employed (51.1%), never received incentives (57.8%), received health care support (53.3%). The result of chi square test showed that there was a correlation between job (p = 0,001), incentive (p = 0,011) with participation of PIS-PK cadres. It can be concluded that the variables of work and incentives are related to the participation of cadres in supporting the PIS-PK program at Puskesmas Gandus Kota Palembang. The researcher's suggestion for the puskesmas is that incentive should be sourced from BOK (Health Operational Assistance) fund from APBD (South Sumatera Provincial Revenue and Expenditure) to be given periodically, it is better to choose unemployed cadres and health workers should help the cadres in the data not just inform the schedule of data collection alone, and preferably Gandus Puskesmas do a lot of training related to PIS-PK to increase knowledge, and motivation of cadres in participating in PIS-PK
