Stroke non hemorrhagic is a condition when there is a dysfunction of neurologic which is showed by loss of motor, sensor, and visual function. Loss of those functions caused the occurrence of self-care deficit independently. The aim of this research is to describe the result of assessment, diagnose, intervention, implementation, and evaluation of nursing care in stroke non hemorrhagic patient with swallowing disorder. The population of this research is stroke non hemorrhagic patients in Oleg Ward, RSD Mangusada. The technique that used to aggregate the data is observation documentation in 2 subjects and was conducted on April 2020. The result shows there is a difference of assessment result on patient 1 and 2 with the theory. In diagnose there is a difference, based on the format that used in the ward haven’t used PES yet. The interventions that already planned in the ward have some similarity, but there is a difference source that used either in the ward and theory. The intervention have not implemented yet totally and the evaluation of patient 1 and 2 has been reached. It is expected the next nursing care documentation can appropriate with the theory.

ABSTRAK


Stroke non hemoragik merupakan keadaan terjadinya disfungsi neurologic yang dimanifestasikan dengan kehilangan fungsi motoric, sensorik, dan visual. Kehilangan fungsi tersebut dapat menyebabkan penurunan kemampuan perawatan diri secara mandiri. Tujuan dari penelitian ini adalah mendeskripsikan hasil pengkajian, diagnosis, perencanaan, implementasi, dan evaluasi dari asuhan keperawatan pada pasien stroke non hemoragik dengan gangguan menelan. Metode penelitian ini adalah penelitian deskriptif dengan rancangan studi kasus dan pendekatan prospektif. Populasi penelitian ini adalah pasien stroke non hemoragik dengan gangguan menelan di Ruang Oleg, RSD Mangusada. Teknik pengumpulan data yang digunakan adalah observasi dokumentasi pada 2 subjek dan dilakukan pada April 2020. Hasil penelitian menunjukkan terdapat perbedaan hasil pengkajian antara data pasien 1 dan 2 dengan teori. Pada diagnosis terdapat perbedaan, yang bersumber pada penulisan di ruangan yang belum menggunakan format PES. Intervensi yang direncanakan di ruangan memiliki beberapa kesamaan dengan teori, namun terdapat perbedaan sumber acuan yang digunakan di ruangan maupun di teori. Implementasi keperawatan belum dilaksanakan secara keseluruhan sesuai yang sudah ditetapkan, dan pada evaluasi pasien 1 dan pasien 2 telah tercapai. Diharapkan pada pendokumentasian asuhan keperawatan selanjutnya dapat disesuaikan dengan teori yang ada.

Diharapkan pada pendokumentasian asuhan keperawatan selanjutnya dapat disesuaikan dengan teori yang ada.
