- Congestive heart failure is a heart failure in pumping blood to adequately meet the body's own nutrient and oxygen needs adequately. Nursing problem that arises in patients congestive heart failure is activity intolerance. According study in Brazil, 25 patients congestive heart failure, 96% experienced activity intolerance which is nursing problem that often arises after decrease cardiac output. Purpose of this study was to determine description nursing care in patients congestive heart failure with activity intolerance in Cendrawasih Room Wangaya Hospital 2018. Type research used is descriptive study with case study design to present-two patients with congestive heart failure observed in depth by observing the patient's nursing document, then comparing the results of patient document observation with reference theory researcher. Data collection tool form study guide documentation. The results showed that there almost similarities in two patients congestive heart failure who experienced activity intolerance in documenting nursing care in room, there some differences from theory used as reference by researchers with results documenting nursing care in room is assessment, diagnosis, intervention, implementation, evaluation. Results this study are expected to provide an understanding nurses in providing optimal nursing care, particularly in provision nursing care in patients congestive heart failure with activity intolerance.
