ABSTRACT
So that a newborn baby weighing less than 2500 grams must be able to maintain a balance between heat formation and heat loss in order to maintain body temperature at normal limits. This tends to cause the baby to experience ineffective thermoregulation and cause hypothermia and hyperthermia. The prevalence of LBW in 2013 was lower than in 2010, the highest was in Central Sulawesi and the lowest was in North Sumatra. The purpose of this study was to obtain picture of nursing care in Low Birth Weight infants with thermoregulation is not effective in the NICU Room at Mangusada Hospital. This research uses descriptive research with documentation study data collection techniques through two patient documents conducted in April 2020. The results of this study indicate that the first and second document assessment results obtained from the patient objective data in the form of patient body temperature data below the normal range, the patient's skin appears reddish. Nursing diagnoses formulated by nurses in the first document and the second is hypothermia. Nursing planning documented using NIC NOC. The implementation included general implementation and there were only three interventions than had been planned. The nursing evaluation used is SOAP documentation. It can be concluded that there are some differences with the theories that have been put forward by researchers both from assessment to evaluation of nursing. This research is expected to be used as material to add insight into science in the field of nursing both for the Polytechnic Ministry of Health of Denpasar and to the Mangusada Regional Hospital.

ABSTRAK
Bayi baru lahir dengan berat kurang dari 2500 gram harus dapat menjaga keseimbangan antara pembentukan panas dan kehilangan panas agar dapat mempertahankan suhu tubuh di batas normal. Hal ini cenderung mengakibatkan bayi mengalami termoregulasi tidak efektif serta menyebabkan hipotermi maupun hipertermi. Pravalensi BBLR tahun 2013 lebih rendah dari tahun 2010, tertinggi terdapat di Sulawesi Tengah dan terendah di Sumatera Utara. Penelitian ini bertujuan untuk mengetahui gambaran asuhan keperawatan pada bayi BBLR dengan termoregulasi tidak efektif di Ruang NICU RSD Mangusada. Penelitian ini menggunakan jenis penelitian deskritif dengan teknik pengumpulan data studi dokumentasi melalui dua dokumen pasien yang dilaksanakan pada bulan april 2020. Hasil penelitian ini menunjukkan pengkajian dari kedua dokumen didapatkan hasil data objektif pasien berupa data suhu tubuh pasien dibawah rentang normal, kulit pasien tampak kemerahan. Diagnosis keperawatan yang dirumuskan yaitu hipotermi. Perencanaan keperawatan yang didokumentasikan menggunakan NIC NOC. Implementasi yang dilakukan sesuai dengan intervensi yang telah dirumuskan. Evaluasi keperawatan yang digunakan adalah pendokumentasian SOAP. Dapat disimpulkan ada beberapa perbedaan dengan teori yang telah dikemukakan oleh peneliti baik dari penilaian hingga evaluasi keperawatan. Penelitian ini diharapkan dapat digunakan sebagai bahan untuk menambah wawasan ilmu pengetahuan di bidang keperawatan baik bagi institusi Politeknik Kemenkes Denpasar dan kepada pihak RSD Mangusada.
