- Caries is a major issue dental health public . One of the bacteria that cause tooth decay is a Streptococcus mutans bacterial , where bacterial growth can be inhibited by the antibacterial agent contained in the red betel leaf ( Piper crocatum ). This study aimed to determine the effectiveness of herbal mouthwash red betel leaf extract ( Piper crocatum ) on the growth of Streptococcus mutans bacteria. Experimental design of this study was pretest and posttest control group design , using ethanol extract of red betel leaf with a concentration of 50 % and 75 % . Subjects in the form of growth of Streptococcus mutans were isolated from plaque respondents, and then the bacteria embedded in media Mueller Hinton Agar ( MHA ). Antibacterial power was observed by looking at the presence or absence of the growth of Streptococcus mutans bacteria in the initial test ( pretest ) and the end of test ( posttest ). The analysis showed Cochran test p-value = 0.044 , we can conclude that the ethanol extract of red betel leaf is able to inhibit the growth of Streptococcus mutans ( p < 0,05 ). It is concluded that there are significant differences in the concentrations of ethanol extract of red betel leaf as compared with 0.2 % chlorhexidine
