- Chronic kidney disease is a disease in which the body's ability fails to maintain the metabolism of fluid and electrolyte balance and leads to death. The type of research used is descriptive research type with case study design. The subjects of the study were 2 patients. Data obtained by means of observation and documentation of patient medical records. The purpose of this study was to describe nursing care in patients with chronic renal failure with hypervolemia that included assessment, nursing diagnoses, nursing orders, nursing implementation, and nursing evaluation. The results of this study were the assessment of both patients complaining of dyspnea, the presence of peripheral edema, oliguria, neck venous distention, decreased Hb, decreased Ht, and positive fluid balance. Nursing diagnoses are hypervolemia. The planned interventions consist of fluid management and fluid monitoring. The implementation only implementation in general and only included three of the planned intervention. Evaluation of nursing documentation method used is SOAP. The result of research indicate a difference with the theory that has been submitted either form the assessment to the evaluation of nursing.
