- The quality of health services in hospitals must be optimally improved, one of which is by completing general consent. Ungaran District Hospital is a hospital that has been accredited plenary, but from the results of the preliminary study, out of 10 (ten) general consent forms there are only 20% (2 forms) which are completely filled out and 80% (8 forms) incomplete. This study aims to identify the factors causing incompleteness in filling general consent using the fishbone method.This type of research is descriptive. The subjects of this study were the morning shift inpatient registration officer and / or patient / family involved in filling general consent. The method used is the study of documentation and observation. Data analysis uses descriptive analysis.The results showed the factors causing incompleteness in filling general consent include human factors in the form of officer characteristics, namely officers having an adult age with multiple workloads, there are officers with a high school / vocational education background, a new employee's service period of less than 6 years. The method is the lack of socialization of SPO filling general consent, the flow of the implementation of general consent filling is not in accordance with the SPO general consent. The machine is torn carbon paper on postek label printers. Material is the lack of use of general consent forms and the lack of effective communication of officers with patients.
