- Research is conducted at home Hospital of Islam Al-Ihsan Baleendah-Bandung at Medical Unit Record of Rawat Jalan with depository and data-processing problems fundamental data into database still not yet integrated in record-keeping of patient data still conducted by manual and use courier medium (nurse) in delivering the the document to each related shares and also there is no reporting facility of information system which can present patient data periodical. Intention of this research is to study Medical Record procedure and system at shares of Rawat Jalan which is walking, and also study and solve problems which is on system. In execution of research use descriptive research method with Observation steps and Interview and also system development method weared at this scheme is method of Prototype and use method approach of system orient or object of OOP (Object of Oriented Programing). Peripheral of supporter the used is UML (Unified Modelling of Language) with a few conducive diagrams to design system Constructively with Rational Rose in depicting the diagram. In developing Information system application used by Ianguage of Borland Delphi 7.0 and SQL Server 2000 as database storage media. The Medical Information System Record at shares of Rawat Jalan problems minimization can related to depository data processing and also data into database which is integrated able to lessen possibility of data duplicate also lessening record-keeping by manual able to result heaping of data and accomodate reporting concerning patient data periodical.
