- Manually recording medical records at the unila polyclinic on paper is one of the main factors of the magnitude of the needs that must be met. Searching for one sheet of medical records from all paper shelves requires a lot of time, therefore it is necessary to create an information system in order to facilitate all recording activities, searching for medical record data, and making decisions for managers. The system development process uses the prototyping method. The research resulted in three cycles, in the first cycle it produced a prototype containing all system requirements, followed by the second cycle which carried out the evaluation requested by the client in the first cycle, and in the third cycle it focused on evaluating and completing the system prototyping. Pencatatan rekam medis pada poliklinik unila yang dilakukan secara manual pada kertas merupakan salah satu faktor utama besarnya kebutuhan yang harus dipenuhi. Upaya pencarian satu lembar rekam medis dari seluruh rak kertas membutuhkan waktu yang banyak maka dari itu dibutuhkan pembuatan sistem informasi guna untuk mempermudah semua kegiatan pencatatan, pencarian data rekam medis, dan pengambilan keputusan bagi manajer. Proses pengembangan sistem menggunakan metode prototyping. Penelitian ini menghasilkan tiga cycle, pada cycle pertama menghasilkan prototipe yang memuat semua kebutuhan sistem, dilanjutkan dengan cycle ke-dua yang melakukan evaluasi yang diminta oleh klien pada cycle pertama, dan pada cyle ke-tiga berfokus pada melakukan evaluasi dan penyelesaian pembuatan prototipe sistem.
