- Bidang pendidikan madrasah merupakan instansi pemerintahan yang berada di Kabupaten Lampung Barat. Pada bidang pendidikan madrasah mempunyai sumber daya manusia meliputi seorang guru. Guru harus meningkatkan kemampuan dan keterampilan dalam menerapkan kompetensi yang diperlukan dalam proses pembelajaran. Penilaian kinerja guru sangat diperlukan untuk mengidentifikasi kemampuan guru dalam melaksanakan tugasnya melalui pengukuran penguasaan kompetensi. Penilaian kinerja guru di bidang pendidikan madrasah masih memiliki beberapa masalah yang dapat menghambat dalam proses penilaian kinerja guru seperti masih menggunakan sistem konvensional, sehingga menyebabkan kendala yaitu menyulitkan proses perhitungan, pembuatan laporan dan rekap penilaian. Pengembangan sistem informasi ini menggunakan metode pengembangan waterfall. Penelitian ini berhasil membangun sistem informasi laporan bulanan dan penilaian kinerja guru berbasis web menggunakan Framework Laravel. Bersadarkan pengujian black box dengan metode equivalence partitioning sistem secara keseluruhan telah berfungsi sesuai yang diharapkan. Oleh karena itu, dengan adanya sistem informasi penilaian kinerja guru dapat menyelesaikan masalah seperti proses penilaian kinerja guru menjadi lebih cepat dan efisien serta mempermudah proses pembuatan laporan. Kata Kunci : Sistem Informasi, Laporan, Penilaian Kinerja Guru. ABSTRACT Madrasah education sector is a government agency located in West Lampung Regency. In the field of madrasah education has human resources including a teacher. Teachers must improve their abilities and skills in applying the competencies needed in the learning process. Teacher performance assessment is very necessary to identify the ability of teachers to carry out their duties through measurement of competency mastery. Teacher performance appraisal in the field of madrasah education still has several problems that can hamper the teacher performance appraisal process, such as using a conventional system, which causes obstacles which make it difficult to calculate, report and recap assessment. The development of this information system uses the waterfall development method. This research succeeded in building a monthly report information system and web-based teacher performance appraisal using the Laravel Framework. Awareness of black box testing using the equivalence partitioning system as a whole has been functioning as expected. Therefore, the existence of a teacher performance appraisal information system can solve problems such as the teacher performance appraisal process which is faster and more efficient and simplifies the process of making reports.
