- Every year a lot of crane accident cause the crane operated with a overload capacity by the company of crane. Even the accident has a lot of factor the one of them is cause human resource is not to well about crane rating chart of the operation instruction. Failure analysis is considered as a part of incident and accident investigation. Its ultimate goal is to improve safety, namely by finding the root cause or contributing factors. The recommendations are to prevent the reoccurrence of the accident / incident. The investigation should reveal “what happened”, “how it happened” and “why it happened”. The investigations can be conducted internally within the company, or by a third party, or by an investigation organization, depending on the nature of the investigation. Regardless who performs it, the investigation should be independent. One of the keywords is objectivity. All parties involved in the investigation should establish this spirit right from the beginning. From the analysis with ansys version 8.0 we get the deflection maximum happen in a range show by the red color with value 0,478x10-6 m, and the blue range is minimum deflection with value 0 m, but the real conditions that range has different value.
