- Hazard Identification Risk Assessement (HIRA) is one method of identifying work accidents with risk assessment as one of the points for implementing occupational safety and health management systems (SMK3). The implementation of HIRA aims to identify potential hazards in a company to assess the probability of an accident or loss and Failure Tree Analysis (FTA) is a technique used to identify risks that contribute to failure. This method is carried out with a top down approach, which begins with the assumption of failure or loss from a top event then details the causes of a (top event) to a basic failure. The HIRA and FTA methods are prepared based on solving the problems that occur in the company are expected to be able to obtain the value of the risk of potential hazards, the categories of potential hazards and the causes of work accidents which are then analyzed in order to obtain suggestions for improvement. Calculation results based on data from 40 occupational accidents from August 2017 to December 2018 obtained from the method of Hira 2, the highest hazard risk value of 18 potential risks and the results obtained from the FTA are the basic factors 18 potential occupational hazards.
