- This studied aims to determine the process of waste, the factors causing waste, and proposed improvements and priority improvements that must do by the Pharmacy Hospital of the University of Muhammadiyah Malang. Muhammadiyah University of Malang Education Hospital is one of a variety of business units owned by the Muhmmadiyah University of Malang. The data used in this study are primary and secondary data, obtained from observation and documentation, namely the time of each process activity and process flow. The sample used in this study amounted to 30 recipes in nonBPJS outpatient services. The sample collection technique used the accidental sampling method. Data analysis techniques used are time function mapping, fishbone, and FMEA (failure modes and effects). The results of this study can be concluded that 6 processes have problems, and which must be corrected immediately, namely the process of making payment receipts
