- PT. XYZ is a company producing white crystal sugar (GKP) Superior Hooft Suiker IA type (SHS IA), where every sugar production process uses dangerous and risky machines which at any time can threaten the safety and health of the workforce. This study aims to identify the potential hazard of workforce accidents and find out the contributing factors, as well as provide suggestions regarding the improvements to minimize the occurrence work accident. Hazard Identification and Risk Assessment (HIRA) is a method which is a method used to identify potential work hazards and the risk values and categories at the preparation station will be identified later. Identification that had been conducted revealed that 10 potential hazard at the preparation station existed with the 4 most common accidents such as slipping on sugar cane and falling from truck, being crushed of sugar cane, falling from stairs, and having short circuit. Then analyzed to find the cause of work accidents using the FTA (Fault Tree Analysis) method which is a diagram model consisting of several combinations of errors in parallel and sequentially which can be caused by several work activities to be analyzed to obtain appropriate suggestions for improvement for the company
