- PT. Wijaya Karya Beton (Pasuruan) is a precast concrete company. At the beginning of the observation, some wastes appear such as 1) defect on the precast concrete piles body, 2) excessive transportation caused by a quite far distance of each line, 3) and inappropriate processing. The waste affecting the appearance of the other wastes. To minimize the waste, identification of dominant waste will be carried out using the waste assessment model’s method. 5 dominant wastes was identified which affecting pc piles production such as defect (25,47%), overproduction (18,81%), unnecessary inventory (15,97%), transportation (13,81%) and motion (11,77%). Afterward, the dominant wastes will be identified using value stream analysis tools. Then, an analysis will be performed to find root causes using the 5 whys analysis method. The final step is the proposed improvement using the failure mode effect analysis method.
