- PT. Fajar Putra Plasindo is a company engaged in manufacturing plastic pallet. In the NS 1210.2-4T plastic pallet production process there are still defective products with the following criteria: short shot defects of 420 units or around 70.6%, color streaks defects of 112 units or around 18.8%, and flashing defects of 63 units or around 10.6%. To overcome a number of types of defects, the Define, Measure, Analyze, and Improve (DMAI) stages are applied with Kaizen as the improve stage. So that in the end it can be known the right proposal to overcome the type of product defects as described above. The recommended action to overcome product defects is based on the value of Cause and Effect at the analyze stage. Where selected 11 causes of disability which are priority causes of disability. Then for the 11 disabilities will be corrected using the kaizen concept that is using 5W + 1H and also 5S.
