- Occupational accidents are accidents related to work in a company, meaning that work accidents are caused by work or when carrying out work. So many dangers can arise from around the workplace. Problems with UD. Jepara is a work accident problem with a number of cases during 2016 there were 72 incidents of workplace accidents. From this data it can be seen that the standards for implementing K3 programs at UD. Jepara is still low. In an effort to minimize human error in work accidents and the dangers posed by the production process there are several methods used are the HIRA and SHERPA methods. Hazard Identification and Risk Assessment (HIRA) is a process carried out to identify all situations or events that have the potential to cause workplace accidents. SHERPA (Systematic Human Error Reduction and Prediction) is a technique to reduce the level of accidents at work, the aim is to evaluate the possibility of human error occurring throughout the completion of certain task.
