- In recent years the industrial sector has evolved and involves a variety of business operations, as well as activities, manufacturing which is considered to be one of the activities that has had many effects on the environment, especially negative effects. The resulting environmental impacts occur during the production process, starting from the procurement of raw materials, followed by the production process, and to the stage of product reuse. CV Surya Food Indonesia is a company that produces various types of bread located in blitar. Based on the initial observations, the company still has not implemented Green Supply Chain (GSC) performance measurement so that the industrial activity occurring in CV Surya Food Indonesia still has an impact on the environment. This study discusses GSCM performance measurement with Green Supply Chain Operation Reference (GSCOR) approach in the company. Performance measurement begins with an initial hierarchy based on processes in GSCOR, ie Plan, Source, Make, Delivery, and Return with common dimensions, Reliability, Responsiveness, Flexibility and Asset all related to the environment. Then the initial hierarchy is adjusted to the company's condition is then performed to measure the company's GSCM performance. Identification of the Key Performance Indicator (KPI) becomes a benchmark in subsequent performance measurements to assist in determining the priority of existing criteria. Measurement of GSCM performance in CV. Surya Food Indonesia produces 23 KPIs and measured by Objective Matrix (OMAX) method which yields the highest importance level value in Reability process. The company's GSCM performance value is 6.314 with the category Traffic Light System (TLS) is yellow which indicates that the achievement of GSCM company's performance is sufficient category.
