- PT. Bambang Djaja which is located in Jalan Rungkut Industrial III, Surabaya is a manufacturing company and engaged in the production of outer transformer. Continuity of production process depends on the reliability of work of employees and equipment. The risk of work hazards often occur will interfere with the smooth running of the production and the level of employee safety. In the industrial environment especially in PT. Bambang Djaja still frequent accidents that cause serious injuries causing loss of working days. Work accident caused due to lack of oversight of the work of the company and still weak level of APD applicability so many employees not wearing personal protection when the production process takes place. Here researchers trying to identify accident causes and aspects, to reduce the rate of accidents of work and know the level/rate of accident in PT. Bambang Djaja. In this study focuses on the production of outer unit transformer which is vulnerable to accidents. The methods used in this study implements accident prevention efforts with risk assessment approach in analyzing occupational health and safety. This method will be knowing the level/rate of accident in pt. Bambang Djaja. The results of the achievement of the implementation of K3 Progam PT. Bambang Djaja was at Range 60%-84% with level 3 (carefully) with a yellow color requirements.
