- This study is a case study at RSI Masyithoh Bangil. Data analysis technique applied in this study was descriptive analysis on input, process and output of the form, procedure and procurement and distribution reports. This study analyzed the effectiveness of the report information value. Based on result of the study and discussion, it can be concluded that the input analysis was not good and the form did not have printed serial number. The process was considered good n controlling authorization of all procedure, yet it was found a failure in making purchase order from pharmaceutical department which actually must be issued by the purchasing department. There was also several inadequate form copies found. The output analysis is considered good, inventory report already met the relevant and reliable criteria of information, it was done manually and also by the help of computer technology.
