Nationally in East Java, an increasing number of people with mental disorders around 2 to 3 percent over the last three years. One of the signs of schizophrenia can be an interruption in the thought process, because the client is not able to process and organize his thoughts, thus resulting in an inability to control emotions and feelings. Until clients sometimes do things that deviate regardless of the surroundings. Methods This case study uses a descriptive design as a design that covers the assessment of one unit of intensive research on the client. The research was conducted in Room 23 Psychiatry RSU dr. Saiful Anwar Malang began on March 21 to 26 2011. Methods of data collection using the interview technique, observation, documentation and study of literature. Validity of test methods of data was done by using triangulation, ie engineering methods, techniques sources, theories and techniques. In the action plan and implementation of nursing actions using Perception Stimulation NOT see the picture. Evaluation results obtained from a decline in symptoms of impaired thought processes. This is reinforced by changes in the client's behavioral patterns.
