- Technology exists to provide easiness of a problem faced by the community. One technology that is widely used and very popular nowadays is the web. With the web, has done a lot of information dissemination such as sales information, services, and even information about health. In this thesis,we created a concept web-based information services using the PHP programming language and MySQL database to facilitate the patient who wants to go to the doctor. Patients can easily stand in line to register online. This information service is very easy to handle front office in a queue of patients and patient registration. With the concept of this service, physicians can perform diagnostics, refer patients to other doctors who are members of the system, and conduct discussions with all users who are members of the system. By using this web tool, doctors can deliver the results of the medical records of patients to physician referral destination more safely and more efficiently.
