Strategy and a good system is needed by companies to improve the efficiency and effectiveness of production. Similarly with PT Yamaha Musical Products Indonesia (PT YMPI), of course, requires a good strategy and systems to increase efficiency and minimize costs to improve company performance. One of the production lines is a Handatsuke Body Flute. This production line is the manufacturing process of the Body and Foot of Flute. On this production line is still often do not achieve production targets due because of inefficiency happened. Therefore, the necessary repairs to make the production process more effective and efficient is Lean Production. This approach focuses any activities that provide value to the customers so as to minimize the use of resources that do not add value to the product. Value Stream Mapping (VSM) and a questionnaire with the Borda method used to identify the dominant waste that occurs, Fault Tree Analysis (FTA) is used to identify the factors that cause waste and Failure Mode and Effect Analysis (FMEA) to determine the most serious problems and suggestions for improvement priority. The result showed that the dominant waste is defect (26.32 %) and waiting (22.13%).
