- Home Industry Kendedes is a company producing shuttlecocks. In the company's handling of the production process which is still far from the principle of cost minimization, cause the waste in the process. Therefore we need a lean six sigma approach to overcome these problems. In the operational stage of lean six sigma using phase DMAIC (define, measure, analyze, improve, control). By define phase using SIPOC diagram, operation process chart, and process activity mapping value element waste for waste waiting 89.98%. and waste defect 89.54%. Phase measure sigma value waste waiting 2,6, while the sigma value waste defect 1,9. Phase analyze using failure mode effect analysis (FMEA) values obtained risk priority number (RPN) for waste waiting 300, and the value of risk priority number (RPN) waste defect 560. Value of the probability of success of the action on fault tree analysis (FTA) for waste waiting, with the addition tool fan 77.6%, while the act of waste defect, which classifies feather by feather stem density 99.5%. Based on the simulation results probability value, cost reductions of waste waiting Rp 5.886.608,67 to Rp 1.318.668, and Rp 1.436.500 if using the actual simulation. While the cost of waste defects decreased from Rp 2.339.906,4 to Rp 11699,53 using simulated probability value.
