- PT. Yanaprima Hastapersada, Limited is a company engaged in the Manufacture of plastic bags and various plastic woven bag as well as the manufacture of cement. The results produced by PT. YPHP widely used in the cement industry, fertilizer, fodder, sugar and chemicals plants and production centers mainly by rice and grain. Products produced include grain white karplas for PT. Bulog, sacks of fertilizer for the PT. East Kalimantan Fertilizer, bag block bottom for PT. Cement Balfour, PT Holcim and others. Based on historical data the company over a period of three months ie October s / d in December 2010 showed that the biggest flaw of the plastic sack order PT. Bulog karplas Bulog contained in 15 Kg pack that is 11.75% of the amount produced. Product defect rate that is greater than 4% resulted in PT. YPHP must replace the defective product with the appropriate product specification. Therefore, product quality improvement projects karplas Bulog 15 Kg pack urged to be implemented and was chosen as the object of research. The main purpose of this study is to improve processes to reduce defects that occur on the production floor. This study applies the approach to six sigma DMAIC cycle one. Define stage to identify problems, measure to determine the issues that affect the quality, analyze contains about fish bone diagram to help find the key issues and Improve the cause of the defect is about FMEA (Failure Mode and Effect Analysis) to assist in determining the proposed improvement alternatives will be granted, this study only focused on repair defects in the CTQ with FMEA method. There is one type of defect is critical to the quality of the printing defect type ngeblok sacks, namely visual products with printing ink is too thick in certain parts. The cause of this defect include ink and solvent composition ratio inadequate, TL (toluene) as a destroyer of ink and EA (Ethyl Alcohol) as the dryer is not functioning optimally leading to a slow drying rate which raises the proposed improvements at this stage given that Improve monitoring of the composition of the material and make a SOP (standard operational procedure) or PK (work instructions) on the comparison of the use of ink and solvent for 1 kg: 3 kg while the ratio of oil solvent / thinner / reducer as much as 8 kg: 1 kg: 1 Kg. By reducing the critical flaw is expected to be able to improve the quality of products produced on the production floor.
