- One of manufacturing trajectory mode is Continuous production system, where production process occurs continuously without stopping. The effect of that continuously process is need a facility like machines process which have high efficiency that optimum for keep the process production running well. To keep the efficiency level of machines process is required to undertaken an accurate maintenance method, so the operation process will be running optimum. This research was done at Department of Production NPK Phonska II, PT. Petrokimia Gresik, that aimed to give proposal of maintenance policy, with calculated value of Overall line effectiveness (OLE) previously, and find out the root causes of losses that happened by using Fishbone diagram. From the result of data calculation was known the value of OLE 56,69 %, that acquired by multiplied the two of OLE parameter, Line Availability (LA) and Line Production Quality Performance (LPQP) with each value 75,33 % and 75,25 %. By using OEE world class approach, we could assumed that the value of OLE not optimum yet. Besides that, we also know the highest losses category is equipment failure with percentage 14,97 %. By using fish bone diagram we know the causes of it such as, machine factor, human and environment factor. By noticed the value of OLE and knowing the root causes of losses, so the maintenance policy proposal that could be given such as : optimizing activity of predictive and corrective maintenance, optimizing process of cleaning by applying the concept of Seiketsu - standarization and Seiso - shine the workplace based on pilar of TPM 5S, increasing skill and soft skill of employee with training, perform the Autonomous maintenance by operator, integrated information flow of maintenance between operator and maintenance department.
