Mobitz Type I Second Degree AV Block in a Patient with Dengue Hemorrhagic Fever at Atma Jaya Hospital

- A 19-year-old woman was admitted to the hospital because of a 5-day history of fever accompanied with headache, nausea, vomiting, and myalgia. Hepatomegaly, spontaneous bleeding, and thrombocytopenia were found. Clinical features of this patient were typical for dengue hemorrhagic fever. Confirm test on dengue blot examinations were positive. On the fourth day after admission, a bradyarrhythmia with stable hemodynamic was detected. Electrocardiogram revealed Mobitz type I second degree AV block. Echocardiogram and cardiac enzyme were within normal limit. This heart conduction abnormality was detected in three days without any symptoms of hemodynamic instability. Electrocardiogram was completely back to normal on the seventh day after admission due to improvement of her clinical status and elevation of her thrombocyte. Mobitz type I second degree AV block in a patient with dengue hemorrhagic fever may be a transient functional impairment of atrioventricular node, in which altered autonomic tone may play a role.Keywords: dengue hemorrhagic fever, bradyarrhythmia, Mobitz type I second degree AV block
