- Background and Objective : The hemodynamic response to laryngoscopy and intubation has been attributed to increased sympathetic activity caused by the upper respiratory tract stimulation. Blood pressure increase 40-50% and heart rate increase 20%. These changes are usually short in duration and well are tolerated by patients without of cardiovascular disease and increase intracranial pressure. Many drugs and techniques have been used to prevent the hyper dynamic response induce by laryngoscopy and intubation. In this study we used Fentanyl 2μg/kgBB plus Deksketoprofen 50 mg as premedications in group A with Fentanil 4 μg/kgBB in group B as premedications to decrease the hemodynamic responses due to laryngoscopy and intubation. The aim of this study is to elicit an alternative alternatif drugs to decrease hemodynamic responses to laryngoscopy and intubation. Methods : After getting the approval from the ethic committee of USU medical school. 60 healty samples were enrolled, men and women, age 16 to 50 years with physical state ASA 1, who underwent elective surgery in Adam Malik General Hospital and several hospital in Medan. Sample were then divided randomly into two groups each with 30 subjects. In controlled randomised double blinded clinical trial. Group A received Fentanyl 2μg/kgBB plus Deksketoprofen 50 mg as premedications and Fentanil 4μg/kgBB in group B. Sistolik blood pressure, diastolik blood pressure, mean arterial pressure, heart rate and respiratory rate were record and measure. All data were analysed by using t- independent test and chi-square test. Result : Sistolik blood pressure in group A and group B decrease respectively 2,6% and 8,8%, diastolik blood pressure increase 7,1% in group A and decrease 6,8% in group B, mean arterial pressure increase 2,9% in group A and decrease 7,3% in group B, heart rate increase 13% in group A and increase 4,2% in group B one minutes after laryngoscopy and intubation. Conclusions : Although both premedication Regiment in this study is effective in decreasing the hemodynamic response due to laryngoscopy and intubation, statistically Fentanyl 4μg/kgBW is more effective in controlling hemodynamic response during laryngoscopy and intubation. Deksketoprofen 50 mg iv showed no effect in decreasing hemodynamic response to laringoscopy and intubation. Premedication with Fentanil 4μg/kgBB iv can cause respiratory depression.
