- Information technology has been being important in many aspects of life, one of them is public service. Public service is one of government function as an attemp to fulfill people needs. SAMSAT office Cilegon is one of public service agencies which always measures the customer satisfaction every year using Public Satisfaction Inde (IKM) by distributing questionnaires manually in a form on a sheet of paper so that errors can occure in the process of dissemination, data collection and processing. To reduce the risk of errors, it needed a web-based application. The metode that used in IKM application design is FAST (Framework for the Application of System Thinking) using PHP and database MySQL as programming language. Result of this research is IKM web-based application based on fuctional requirements and non-functional requirement, Data Flow Diagram (DFD) and Entity Relationship Diagram (ERD) on the IKM web-based application design in SAMSAT UPTD office Cilegon.
