- Wire rod mill (WRM) is one of the factories in XYZ company that produces wire rod. Throughout the years 2015 until 2016, the wire rod mill have some problems that cause the production process nonconforming product specifications and production delay so that the number of production declines. The methods used to solve these problems is six sigma method. Moreover, these problems will be followed up with further analysis of FMEA techniques, namely the Multi Attribute Failure Mode Analysis (MAFMA) with Gray AHP approach. This study aims to determine the ability of wire rod production process, then was analyzed potential failure in the production process. Thereafter, it will be given the proposed improvements to the failure of the process. The research design used in this study is a quantitative approach with type of research is a cross-sectional study. Based on the research results, the sigma value obtained is 4.297503 σ with DPMO value is2928,626. The next stage is to identify the root causes of process failure on the basis of disability with the principle of pareto diagram. Potential causes that led to the failure of the process is a hot button bearing roll. The proposed improvements to the five highest of the causes of process failure is to improve the state of bearing roll, roll entry, hydraulic valve, an improvement on roll adjustment and improvement of inter-roll partings. It was concluded that the potential causes that the hot button bearing roll has the highest weight is 0.94569, so making it a top priority improvement.
