- PT Y is a company engaged in the production and distribution of office furniture products. Production system applied in PT Y is make to order that is based on consumer order. An increasingly growing business activity can allow for a risk to occur. Risks that can threaten the business continuity of the company are disruptions that can occur in supply chain activities. To manage the existing risks, supply chain risk management is required. Supply chain management consists of stages of risk identification, risk analysis, risk evaluation and risk mitigation. Model Supply Chain Operations Reference (SCOR) is used to map the company's supply chain activity. Activity mapping makes it easy to identify risk events and risk agents. The risk events and risk agents identified are then assessed for impact level, frequency level and correlation using the House of Risk method. The House of Risk method has two phases. HOR phase 1 to determine the priority of risk agents and HOR phase 2 to determine priorities for mitigation actions. There were 35 risk events, 4 on the process plan, 5 in the source process, 15 in the make process, 7 on the deliver process, and 4 on the return process and 28 identified risk agents. There are 12 priority risk agents and 12 mitigation actions to address priority risk agents.
