  • Lean manufacturing can be defined as an approach to identifying and eliminating waste (waste). PT. Indah Kiat Pulp and Paper is a company producing a white paper, brown paper and a variety of processed products such as paper and cardboard box. Based on observations and interviews on the offset printing obtained waste that occurs is a defect, transportation and waiting. This resulted in the production quantities are not optimal. The purpose of this study was to determine the percentage of each waste, waste most common (dominant) and the proposed improvements are being made to minimize the dominant waste. The method used is a waste assessment model is a method to identify the relationship between waste and waste that occurs with the dominant matrix and questionnaires. Proposed improvements were made based on the method of Failure Mode and Effect Analysis (FMEA). The results obtained percentage of each waste was 16.6% for the overproduction, inventory 15.5% to 19.8% for the defect, 14.9% for the motion, 11.9% for transportation, 9.9% for overprocessing and 11.4% for waiting. Waste dominant defect of 19.8%. Proposed improvements made to minimize waste defect based FMEA is to check the position of the sheet before entering the die cut machine and immediately make improvements sheet position if it does not fit.
