  • Diabetes mellitus requires lifelong treatment to control patient blood sugar levels, resulting in increased therapeutic service costs to improve the patient's quality of life. The purpose of this research was to identify the cost of illness in Type 2 Diabetes Mellitus (T2DM) patients with insulin therapy. This study was conducted from January to March 2019 in 15 community health centers in Surabaya. The respondents were T2DM patients who get insulin as monotherapy or combination. This research was a cross-sectional study with total sampling technique. The perspective used in this research was societal perspective. The costs component were direct medical cost consisting of drug costs, doctor consultation fees, laboratory examination fees, blood sugar test costs, and the cost of drug side effects, direct nonmedical cost consisting of transportation costs and administrative costs, and indirect costs which was the loss of productivity cost. Results showed that the average value of the cost of illness in patients with T2DM with insulin therapy in 15 community health centers in Surabaya was Rp6.496.186 ± 4.048.491 per patient per year. The largest cost component was direct medical costs with a percentage of 85.53% of the total cost of illness. Thus, the results can be used to assist in improving health care policies as well as health insurance for the community.
