- Background: Albumin is a plasma protein that 80% contributes to maintain the capillary oncotic pressure. Hypoalbuminemia is a complication occured in various states of internal medicine that results in extravascular fluid accumulation, hypovolemia, and decrease in protein-drug binding. Giving exogenous albumin should be a consideration because it has restriction on its controversial usage. Objective: The aim of this study is to analyze the utilization and identify drug related problems (DRPs) of albumin in internal medicine patients that had been hospitalized. Method: It was an observational study with retrospective data collection, analyzed descriptively, based on medical records of internal medicine patients that had been hospitalized in RSUA Surabaya from January, 1st to December, 31st 2017. Result: Total sample obtained was 42 patients with 59 times of albumin administration. The major hypoalbuminemia etiologies were sepsis (47.62%), malignancy (30.95%), gangrene (28.57%) and nephropatic diabetes (23.81%). The available albumin in this study included 100mL albumin 20% (69.49%) and 25% (30.51%) administered by slow drip infusion not more than 4 hours. The amount of albumin administration was 1 flask/week (73.81%), 2 flasks/week (14.28%), 3 flasks/week (9.52%), and 4 flasks/week (2.38%). Average increase of albumin level in patients was 0.25±0.40 g/dL and normal albumin concentration was achieved in 4.76% patients. There was no adverse effects found in this study.
