- PT. Intens Abadi is a private company engaged in the manufacture of medical devices with manufacturing specification implans used as a tool to connect and help the recovery of a broken bone. Complex problems come from the production, namely the division of miling and cutting products. Some of the various types of manufacture, the type most widely produced is cancellous screw type Screw full and half and Cortical screw. This study uses three stages that define, measure, analyze, improve and control. In the define phase diagram using SIPOC (Suppliers, Inputs, Process, Output, Customer). This chart provides information on prepared acting as a supplier, what the input, what is the process, what is produced and who the customers or users of goods. Next is a measure to determine CTQ stage then performs defect grouping into three groups, namely crack defects, defect size / tilt, defect chunk. Then determine and calculate DPMO sigma capability. From the calculation of total disability resulting product cortical screw at 24 967 and the average DPMO amounted to 54 529. As for products cancellous screw has a total of 41 765 product defects and average DPMO amounted to 59 925. Further into the analyze phase. In this section we have identified that there are three root causes of problems that produce defect, namely machine, man and mother nature. From the analysis of the root of the problem, it is suggested that the company made some changes so that the whole production process can be well controlled, and can reduce the number of defects on the next production.
