- The number of cases of traffic accidents every year is increasing as more and complexity of the problem of transportation in Indonesia. Mojokerto city is one of the cities with high mobility. Number of accidents recorded in January - February 2016 occurred 133 accidents with 37 victims died, one had severe injuries, 142 minor injuries. A model of traffic accident prevention was initiated by William Haddon Jr., known as the Haddon's matrix. The purpose of this study was to develop a system information of accident risk factor in health department of Mojokerto City. The design was actions research in the form of system development. Researchers analyze the present information system, analyze the problems and obstacles that occur then compose a new design to solve the problem. The new design was conducted trial, then it revised that eventually earned the final design of information systems which is ready to use. This research was conducted in the Health department of Mojokerto City during February to July 2016. Data collected by indepth interview on several informant. Based on study result revealed that in Health Department of Mojokerto City found no information system of accident risk factor, there is only a system for recording and reporting traffic accident cases. Based on the test results of the questionnaire, all trial participants of traffic accident risk factors information System stated that in order to perform data entry on a form available on the web is quite easy and helpful.
