- The problems of immunization program in Blega public health center besides low coverage were problems in the information system of the immunization program that caused the quality of the generated information not valid. This study aimed to develop a database model of recording and reporting of complete basic immunization of infants and advanced of toddlers in Blega PHC. This was a design development with action research. Stages in the research included activities analyzing running system, identifying the data and information needs, design a database model, and test the database model that has been developed. The result of analyzing stage of running system was found the problem on input, process and output component. Identification of data and information needs needed by system could not be produced on the running system. Database design of complete immunization and advanced of toddler used Epi Info Version 7. Based on database trial, database developed obtained results that data base was simple and easy to understand, variables that already meet the needs of the data and information needed, easy to operate and the quality of the data produced by the database was accurate. Implementation of databases must be supported with computer facilities with specifications, human resources who can operationalize database, maintenance management system and training for officers to boost their skills.
