  • Aerobic exercise has been recommended and proven to improve cardiorespiratory fitness in individuals post-stroke, but its effect on walking speed varies. In Guidelines for Diagnosis and Therapy in IRM dr. Soetomo general hospital, aerobic exercise has not yet become part of routine exercise in patients after sub-acute ischemic stroke. The purpose of this study was to compare the effectiveness of the addition of aerobic exercise using a static cycle to routine functional exercise according PDT, on cardiorespiratory fitness and gait speed of patients with sub-acute ischemic stroke. This pretest-posttest control group design research has been performed on 18 patients (9 in the treatment group and 9 in the control group), which underwent exercise 3 times a week for 12 sessions. Subjects in the treatment group received routine functional exercise according PDT, customized with individual impairment, and aerobic exercise using a static cycle with intensity : THR = HRR (40-70%) + HRrest, for 30 minutes. Subjects in the control group only received routine functional exercise. This research has been approved by Health Research Ethics Committee of Dr. Soetomo Hospital. There were improvements in cardiorespiratory fitness after the training program in treatment group, with significantly decreases in resting heart rate (p = 0.002), but there was no significant decrease in the control group (p = 0.280). No significant difference in the reduction in resting pulse rate between the two groups (p=0.171). There were no significant increase in self-selected gait speed in the treatment group (p = 0.069), the control group (p = 0.802) and between the two groups (p = 0.183). Similarly, there were no significant increase in fastest gait speed in the treatment group (p = 0.121), the control group (p = 0.494), and between the two groups (0.291). The effect of adding aerobic exercise was greater in subjects with 2-4 weeks of stroke onset. It was concluded that the addition of aerobic exercise on the routine functional exercise according PDT has the same effectiveness with routine functional exercise according PDT in improving cardiorespiratory fitness after stroke as well as in increasing the self-selected gait speed (SS) and the fastest gait speed (fast pace).
