- In operational activities, a health service requires an information system to collect, process and store data and distribute an information, in which the information continues to grow and the number increases. Information Systems is a combination of human, technological facilities, tools, media, procedures and controls aimed at organizing critical communications networks, specific transactional processes and routines, assisting management and internal and external users and providing the basis for appropriate decision making. Based on the observation in Unpas Clinic, it shows some problem that is patient data which previously registered suddenly lost so that must fill new data, patient data still present in patient status card, when data input have not done digitally. From some of these problems it is necessary to design an information system to improve accessibility of data presented in a timely and accurate manner, without requiring the existence of information system intermediaries, ensuring the availability of quality and skills in utilizing information systems critically, developing effective planning process, The need for information systems support skills, establishing investments that will be directed to information systems, anticipating and understanding the economic consequences of new information systems and technologies, and improving productivity in systems development and maintenance applications.
