  • Supply chain activities are inseparable from the opportunities for risk arising, therefore risk management is needed in handling risks. The aim is to minimize the level of risk and the impact of these risks. PT. Pupuk Iskandar Muda is a subsidiary of PT Pupuk Indonesia (Persero) which is engaged in the fertilizer industry. From direct observation and interviews, it was found that disruption in the company's supply chain requires good supply chain planning, including identifying the risks in the supply chain, determining the causes of dominant risks and planning a number of risk mitigation actions. An analysis and evaluation of risks that have the potential to emerge in a company's supply chain uses the House of Risk (HOR) approach. Mapping supply chain activities based on the SCOR model is then given a severity, occurrence and relationship assessment of risk events and risk agents to get the aggregate risk potential (ARP) value. Then a number of mitigation actions are planned and assessed the level of difficulty and their relationship to the dominant risk causes to get priority risk mitigation actions. After doing research there are 42 risk events and 42 risk agents. With the 80/20 pareto approach, 9 risk agents are planned for which mitigation actions are planned. There are 11 proposed risk mitigation actions that are recommended to companies in the hope of managing the risk of urea fertilizer supply chain.Keywords: supply chain, House of Risk (HOR), Aggregate Risk Potential (ARP), risk mitigation, mitigation action
