- The purpose of this study was to determine the cause of product defects and using Fault Tree Analysis ( FTA ) in Sidoarjo Industry Medica PT.Jayamas. By using the method of Fault Tree Analysis ( FTA ) , to determine the issues and make suggestions for the improvement of prevention and reducing the potential cause of disability products . The results of the analysis showed that the type of defect does not include Nepkin Peaper Precision probability 0.1742 ( 17.42 % ) where the root - the root cause is the lack of precision Peaper Nepkin position , the motor pengerak nepkin peaper unstable , roll bearing wear , nepkin peaper folding , and operator less scrupulous . Packing Seal Leaking probability 0.163 ( 16.3 % ) where the root - the root cause is the lack of skilled operators , machine settings are less precise , less roll adhesive roll heat and wear . Position Logo Less Precision probabilities 0,139 ( 13.9 % ) the cause is Sliding oblique roll , blunt cuter , less precision positioning cutter , napkin peaper robeh and less scrupulous operators . Sandwiched Tsu probability 0.0345 ( 3.45 % ) . Seal Scorched probability 0.0054 ( 0.54 % ) Proposed improvements to the company based Correction Action performed on the type of defect that has more than a 10 % probability that the type of disability Nepkin not Peaper Precision , Seal Leaking Packaging , Logo Less Precision Position .
