  • Osteoarthritis (OA) is one of the most common encountered musculoskeletal diseases. Pain is a major complaint and that is often delivered by OA. Pain in OA is a degenerative joint pain that occurs due to a combination of various factors, one of them is inflammatory. The study has shown an increased activity of COX enzymes in the inflammatory process and pain of joint diseases. The purpose of this study was to determine the correlation of the degrees of pain assessed by visual analogue scale (VAS) and the ability of secretion of cyclooxygenase-2 (COX-2) monocytes in synovial fluid of patients with knee OA knee. This study is a cross-sectional study with research subjects are people with knee OA in the department of internal medicine clinic of RSUP Dr. Sardjito. The patients, who meet inclusion and exclusion criteria, are taken to assess knee joint fluid secretion of COX-2 and the degree of pain. Secretion of monocyte COX-2 was observed under light microscope. Cytoplasms of cells, that do not secrete monocyte COX-2, are white, while the COX-2 secretes brown. The correlation test of Spearman is performed to analyze the correlation of the degrees of pain with the ability of COX-2 secretion of synovial fluid monocytes. The level of significance is p < 0.05. The study found that 67 subjects were consisting of 22 (32.8%) men and 45 (67.2%) women. The mean (± SD) of VAS score was 52.41 ± 24.53 mm and the mean (± SD) of COX-2 secretion of synovial fluid monocytes was 0.84 +0.37. the study found that the correlation of the ability of COX-2 secretion of synovial fluid monocytes and the VAS score was r = 0.176 with p = 0.153 < 0.05. The conclusion of this study is that there is a non-significant positive correlation of COX-2 secretions of monocytes and the degree of pain in patients with knee OA.
