- Background: Diarrhea is a major health problem in Indonesia. The prevalence of diarrhea in underfives is 31.4%, and mortality rate caused by diarrhea is 25.2%. This figure is higher than pneumonia (15.5%), dengue hemorrhagic fever (6.8%), measles (5.8% (Riskesdas 2007). At the Province of Bengkulu the prevalence of diarrhea is still relatively high. In 2009 there were 32,940 cases and in underfives there were 18,181 cases (64.37%). According to data of Bengkulu Municipal Health Office cases of diarrhea from 2006 to 2009 tended to increase. Objective: To identify risk factors of environmental health facilities (type of clean water resources, availability of clean water, utilization of clean water, availability of family toilet, utilization of family toilet) and mothers' behavior (habit of baby/child feeding outside the house and habit of personal hygiene) and the prevalence of diarrhea in underfives at Pasar Ikan Health Center Subdistrict of Teluk Segara Bengkulu Municipality. Method: The study was analytic observational that used prospective case control study design. Samples consisted of 140 people, comprising 70 cases and 70 control. The independent variables were environmental health facilities (type of clean water resources, availability of clean water, utilization of clean water, availability of family toilet, utilization of family toilet) and mothers' behavior (baby/child feeding outside the house and personal hygiene)
