- Background. Laryngeal Mask Airway (LMA) Proseal has bigger dimension and soft consistency compare with other supraglotic devices. This Characteristic allied to better sealing on positive pressure ventilation and airway protection but in the other hand the difficulties of it�s insertion is increase. With the first time successful insertion rate of 80% on digital standard insertion technic is not satisfied enough for such a quick and simple application devices. The application of jaw thrust on airway patency is well understood in anesthesiology field, but It�s effect on the succes of LMA Proseal insertion is not known. This study aim to test the hypothesis that the first attempt success rate of LMA Proseal insertion is higher in jaw thrust technic compared with digital standard technic. Methods. A prospective, randomized open trial study was performed in 80 healthy patients (ASA I or II) undergoing general anesthesia. Patients were randomly allocated into one of two groups to receive jaw thrust technic as group J, with subject of 40 patients and digital standard technic as group S, with subject of 40 patients. The first attempt success rate was compared between the groups. The speed of insertion were also assessed. Result. The first attempt success rate is defined as a succesfull insertion to maintain airway patency, with no repositioning and effective ventilation. First attempt success rate were significantly higer in jaw thrust technic (9.5%) than digital standard technic(72.5%)(p<0.05). The speed of insertion is significantly faster with jaw thrust technic (6.15 ± 1.33) seconds than digital standar (17. 25 ± 4.99) seconds. Conclusion. The First attempt success rate with jaw thrust technic insertion (92.2%), is higher than digital standard technic (75%) and statisticly significant (p<0.05).
