- Background: There was an increasing demand for health information and the usage so widely that the information must be supplied in reliable, accurate and timely. Hence the government issued the policy and strategy to develop electronic government (E-gov) by President Instruction No. 3/2003. To anticipate that issue, since 2005, East Java Provincial Health Office had applied and developed web based information system. One of the them is web based immunization routine reporting system and developed since 2007. Due to this system, all data could be integrated from district health office to provincial health office. But untill now, the system had never been evaluated for the management, problems, succes, accuracy, timeliness, completeness and its usefullness. Research purpose is to evaluate the management and usefullness of web based immunization routine reporting system in east java provincial health office. Objective : to gain the description of web based immunization routine reporting system management due to the technical, organizational, behavior, data quality and their usage. Methods : qualitatitive research, case study, purpossive sampling. Subjects : for qualitative data were gained from four districts with individual data : Jember, Lumajang, Blitar and Ngawi and Provincial Health Office, while quantitave data gained using closed questionnaires that sent to all district/ city health office in east java. Results and Discussion : Web based immunization routine reporting system has four modules and presents to the end user in various ways
