- Hemodialysis is one treatment that can be given to patients with end stage renal failure. Patients with hemodialysis often experience complications that require treatment with medication. Providing a variety of therapies at risk for the occurrence of drug-related problems. This study aims to identify the types of DRPs that occur in hemodialysis patients, knowing the percentage of each incidence of DRPs, knowing the outcome of therapy clinical comorbidities in hemodialysis patients. The study was conducted using an observational study design with prospectively data collection in hemodialysis patients. The population in this study was all patients undergoing hemodialysis in the period May to July 2010. Data obtained from medical records and interview notes with the patient or their family then analyzed descriptively to identify the types of DRPs and the percentage of DRPs occurrence. Total of 151 patients undergoing hemodialysis are 131 patients met the inclusion criteria. The incidence of DRPs occurred in 88 patients or by 67,17%. Type of DRPs occurring untreated indication of 2,17%, sub-therapeutic doses of 21,01%, drug overdoses of 5,79%, adverse drug reactions of 2,90%, failure to receive medication at 57,97% and 10,15% for drug interactions. From the results of blood pressure measurements predialysis 67,18 had not meet the target and 75,57% of the patients blood pressure postdialysis patient had not meet the target, while based on examination of hemoglobin found 31.29% of patients with hemoglobin in accordance with the target and 68.70 % of patients had not reached the target.
